Responsibilities

Pay Range: $50,000 (minimum) to $60,000 (maximum)

$2,500 SIGN-ON BONUS

About the Position:

Coordinates all RTC referrals from referral to admission; Mountain View Place (general medical model RTC for children/adolescent males and females). Maintains organization of all referrals and works in collaboration with other hospital departments to ensure a timely referral process. Collaborates with referral sources, families, doctors, therapists, managed care companies, and agencies to gather all appropriate clinical information for RTC admissions. Responsible for developing, implementing, and maintaining the completion of admit paperwork for all direct admits to all acute units.

Cedar Springs Hospital COVID-19 vaccination mandate:

In accordance with Colorado state public health order, all healthcare workers must be vaccinated, and the Company must verify vaccine status. Pursuant to this public health order, the following modes constitute as proof of vaccination:

      • an electronic entry in the Colorado Immunization Information System (CIIS); OR
      • an immunization record from a licensed healthcare provider who has administered a COVID-19 vaccine to an individual who provides services for the facility or agency, specifying the vaccine administered, name and title of the person who administered the vaccine, address of the location where the vaccine was administered, and the date it was administered.

A COVID-19 Vaccination Record Card (issued by the Department of Health and Human Services Centers for Disease Control & Prevention or WHO Yellow Card) which includes name of person vaccinated, type of vaccine provided, and date last dose administered would suffice.

Exemptions

  1. Workers may be exempt from the vaccination requirements by providing a signed exemption form indicating one of the following:

  1. Religious Beliefs
  2. Qualifying Medical Reason

  1. To be eligible for an exemption based on Religious Beliefs, the worker must provide a written statement describing the worker's sincerely held religious beliefs and how those beliefs conflict with Colorado's August 21, 2021 Public Health Order requiring certain healthcare workers to be fully vaccinated against COVID-19. The company may request supporting documentation to further evaluate the request for religious accommodation.

  1. To be eligible for a Qualified Medical Reason exemption, the worker must provide a written statement signed by a physician, physician assistant, advanced practice nurse, or certified nurse midwife licensed in the State of Colorado stating that the COVID-19 vaccination for the employee, direct contractor, or support staff is medically contraindicated as described in the product labeling or authorized by the FDA. This statement should not describe the underlying medical condition or disability but should indicate the probable duration of the inability to receive the vaccine.

  1. A record of signed exemption forms and statements submitted in support of the exemption request must be maintained in a confidential HR Medical file, separate from the Personnel File.
Qualifications

Physical Job Requirements:

Position requires sitting for prolonged periods of time.

Occasionally lifts up to 10 pounds and carries small objects from waist to chest.

Must be able to type and use a computer/excel spreadsheets

Ability to distinguish telephone rings and to hear and speak clearly over the telephone.

Regularly needs to bend, stoop and reach.

Must have a valid Colorado driver's license. Must be able to drive a vehicle if necessary.

Must be able to speak, read and write.

Emotional stability necessary to perform the essential functions of the job.

Vision and hearing sufficient to perform the essential functions of the job.

Skills

Requires organizational, verbal, interpersonal and customer service skills, grammar and spelling, the ability to read/comprehend written instructions and follow verbal instructions. Ability to establish and maintain effective working relationships with employees, managers, referral sources, patients/families, community resources, etc. Ability to exercise professional independent judgment in planning own work. Must be flexible with work hours to accommodate work volumes. Ability to multi-task and stay calm when dealing with multiple issues. Excellent time management skills, able to work in a fast paced environment, multitask proficiently, professional and clinical communication styles, and excellent documentation skills in multiple computer logs. Must be able to work independently and be self-driven.

Experience/Education

Bachelor's degree in the behavioral sciences required. Knowledge of insurance/managed care/Medicaid/MHASAs perferred but not required. Clinical experience preferred but not required. Minimum of one year experience in mental health and/or an admissions department in a health care setting.

Staff Development Requirements:

Must meet or exceed hospital and regulatory agency training requirements.

Exposure Category: III

The normal work routine involves no exposure to blood, body fluids, or tissues.
